The recent revival of Three Tall Women (spoilers, maybe?)


had the ushers hand out a playbill insert page *after* the performance to acknowledge the actor playing The Boy, I suppose to keep his appearance a surprise. The actress who played A lying in bed during the second half was not credited in the playbill or elsewhere. A different situation, but the point is I think there is some flexibility in crediting actors. I'm sure there are other examples too.
