Part of this is on the theater owners. Case in point: I worked at the Nederlander for many years during RENT. Prior to RENT this house was frequently vacant (mostly due to an 'undesirable' location south of 42nd Street). The producers of RENT wanted a 'distressed' feeling FOH and stopped the Nederlanders during their spit & polish reno for their new tenant (hence the FOH scaffolding left in place during the entire 12 year run). I digress. When curtain came down and the ushers thrust open the doors to the street to alleviate foot traffic in the lobby, I remember those doors being a solid 6+ inches thick (wood). Surprisingly not weighty, but efficient with noise reduction (not that RENT needed it - when folks complained the show was too loud, they were routinely offered disposable ear plugs). After RENT closed, the Nederlanders did a proper reno of their namesake house and at the next show I worked on 41st Street I noticed that those thick doors exiting onto the street were replaced with commonplace hope-nobodys-standing-on-the-other-side doors. I'm sure there are fire regulations that enter into these decisions (fire exit doors probs must be metal), but the theater owners bear some of the burden for noise reduction.
