I saw the show on Saturday evening with Julia Chan in the role and she also held a script. I read a review afterwards that said having "A" carry the script was part of the concept of the director, Maria Dizzia; Tatiana Maslany also did it at the opening. It probably is partly a safety measure given that the various actresses rotating into the role don't get a lot of rehearsal time, but I think it's also meant to convey the universality of the story of violence, how it can be passed on to different women who each have to carry it and cope with it, and it's left onstage at the end of the play for the next woman to pick up.
Chan didn't prop up her leg though, so my guess would be that O'Connell has a real injury.
