Several years ago, the restrooms in the college building that I teach in (and many others on campus) were converted to unisex use, with big signs on the doors. I'm not aware of anyone complaining, and for what I can tell, it's, um, business as usual. But the difference here is that the restrooms at the Sondheim Theatre are NOT marked as unisex, and for an attendant to suddenly decide, mid-stream, as it were, to change that, could upset some people who would be less comfortable in a unisex situation.

And yes, it's not always about anatomy per se, but about the privacy of the given body functions in general. For some people, as mknyc76 said, it's tough enough to do that with others right there next to you, even if they're the same gender. To be thrown a sudden curve that both genders are all of a sudden going to be sharing isn't a trivial thing.
